Making An Appointment

New CommUnityCare patients should call our Patient Access Center at 512-978-9015 to be registered in our system and to select the location that will become your medical home. Prior to becoming an established patient, you should speak with an elegibility specialist to determine which programs you qualify for. The following link will provide you with more elegibility information:

Once you become an established patient at one of our sites, you may be asked to call that site directly for future appointments. Main numbers for our individual clinic sites can be found under "clinic locations".

If you seek care at another health center or hospital, please provide them with your personal CommUnityCare clinician's name and phone number. Click here for a listing of our clinics.

After Hours/Emergencies

If you are experiencing a health problem after hours, our Hancock and William Cannon locations have extended hours for walk-in care. These services are not meant for someone experiencing a medical emergency.  Hancock is open every day from 8 am to 8 pm. William Cannon is open Monday through Saturday from 7:15am to 8pm, and is closed on all major holidays.
