Delta Dental

Thank you to the Delta Dental Community Care Foundation for supporting CommUnityCare’s safety net dental program. With funding from this grant, CommUnityCare has updated our dental equipment to better suit our growing patient population, and provide comprehensive dental care to the underserved in Travis County. We are extremely grateful for Delta Dental’s support.

Episcopal Health Foundation

CommUnityCare is a proud grant recipient from the Episcopal Health Foundation. We are grateful for the support from the Episcopal Health Foundation to pilot initiatives aimed at health and not health care. With generous funding from Episcopal Health Foundation, CommUnityCare is building a strong model for team-based care, helping empower patients to manage their social determinants of health, and piloting a program to support access to food that can help prevent the onset of disease for pre-diabetic patients.

Moody Foundation

The Moody Foundation has generously provided CommUnityCare with funding to provide enhanced access to women’s health services in underserved areas, including Bastrop County and the Travis County Eastern Crescent. A grant from the foundation funded the purchase of much-needed equipment to improve patient safety and to expand our capacity to serve, including new access for cervical cancer screening and diagnostics.

St. David’s Foundation

St David’s Foundation generously provides funding for individuals in need of vision services, including eye exams, glasses, or diabetic retinal screening. Patients may request a voucher from a clinical community partner and redeem it for services at over 40 participating vision service providers. A list of participating providers is given to patients with the vouchers issued by community partners.

HIV Prevention and Care

HIV Opt-Out Screening

CommUnityCare receives funding from the Department of State Health Services (DSHS) to support the Routine Opt Out HIV Screening Program. With this funding from the State of Texas DSHS, CommUnityCare is able to provide routine HIV screening for patients between the ages of 15 and 65 seen at our health centers. These funds help our health centers remain at the forefront of our community’s efforts to end the HIV Epidemic.

Ryan White Program

Through the support from the City of Austin and the Brazos County Council of Governments, CommUnityCare receives Ryan White HIV/AIDS Program funds. The Ryan White HIV/AIDS Program provides HIV care services for persons residing in Travis, Bastrop, Blanco, Burnet, Caldwell, Fayette, Hays, Lee, Llano and Williamson County who meet the income guidelines and requirements and do not have sufficient health care coverage or financial resources. HIV State Services

The Brazos County Council of Governments provides CommUnityCare state funding to support HIV care services. Texas Health and Human Services Commission (HHSC)

The Texas Health and Human Services Commission (HHSC) funds multiple programs administered by CommUnityCare. Patients can schedule an appointment to check for eligibility in this program by calling 512-978-9015 and requesting an eligibility appointment.

Family Planning Program – The Family Planning Program helps CommUnityCare provide quality, comprehensive, low-cost, and accessible family planning and reproductive health care services to women and men.

Title V – With the Title V program, CommUnityCare provides quality and low-cost preventive medical and dental services to children and pregnant women.

Primary Health Care – With the Primary Health Care Program, CommUnityCare provides quality and low-cost primary care services to women, children, and men.

Community Mental Health Services – With the Community Mental Health Grant, CommUnityCare provides behavioral health services in the same setting as primary care services.
