Existing Patients

Make an Appointment

If you are an established patient, you can use MyChart to make an appointment. Through MyChart, you do not need to call to make appointments, check your results, or even communicate with your provider. To log into MyChart and create an account, click HERE. If this is your first time creating a MyChart account, click HERE for step-by-step instructions.

New Patients

Making A First-Time Appointment

If you are interested in becoming a CommUnityCare patient, there are two ways to make your first appointment:

  1. Online – If you have never been a patient at a CommUnityCare clinic or you have never been to one of our vaccination clinics, click HERE to schedule your first appointment with a CommUnityCare provider. This is the fastest way to schedule an appointment.
  2. Via Phone – You can call our Patient Navigation Center at 512-978-9015 to be registered in our system and to select the location that will become your medical home. Prior to becoming an established patient, you should speak with an eligibility specialist to determine which programs you qualify for. You can do this by calling our eligibility team at 512-978-8130.
