In order to fulfill its requirements of a Federally Qualfiied Health Center, CommUnityCare must define and annually review its federally approved service area including the identification of the medically underserved population or populations within its service area.This review includes an assessment of zip codes that have been approved by the HRSA as part of CommUnityCare’s service including:
- The size of CommUnityCare’s service area to ensure that services provided through our sites are available and accessible to the residents of the service area;
- The boundaries of CommUnityCare’s service area to ensure that it conforms, to the extent practicable, to relevant boundaries of political subdivisions, school districts, and areas served by Federal and State health and social service programs; and
- The boundaries of CommUnityCare’s service area to ensure that it eliminates, to the extent possible, barriers resulting from the area’s physical characteristics, its residential patterns, its economic and social groupings, and available transportation.
