The Provider will participate in the formal process through which the Community Care Collaborative or its designee collects, verifies, and evaluates the professional credentials and qualifications of licensed individual providers against the criteria, standards, and requirements established by the District for providing health care services to Eligible Patients. Provider Credentialing for MAP is handled by Seton Health Plan.
