As a reminder, our primary focus is to serve the uninsured, underinsured, homeless and the most vulnerable, please keep that in mind when trying to establish care with us.

If you seek care at another health center or hospital, please provide them with the name of your CommUnityCare provider and phone number. Click here for a listing of our clinics.
