What is MyChart?

CommUnityCare Health Centers is proud to introduce a new medical record system that will bring many new benefits to you, the patient.

Part of this system is an application called MyChart. This is an easy-to-use secure application that gives you access to your health information. With MyChart, you can view your medications, test results, health summary and immunizations, as well as access other services such as requesting prescription renewals and communicating with your care team.

Features

Appointment Scheduling

Schedule an appointment or request to have the clinic contact you. 

Medication Refills

Ask for a prescription renewal and receive notification when your medications are ready to be picked up at the pharmacy.

Video Visits

Meet with a doctor over video to receive care from the convenience of your home or work.

Test Results

Receive timely notification of results for tests you’ve had done and view test results from the past.

Preventive Care

Keep track of routine care that is due and receive reminders for flu shots and other immunizations, physical exams and preventive care procedures.

Signup for MyChart

By Computer

  1. Go to www.mychart.com/LoginSignup.
  2. Click on Access MyChart.
  3. Choose Texas for where you receive care.
  4. Choose CommUnityCare as your provider.
  5. Select SignUp to create an account and request an activation code.

By Mobile Phone

  1. Go to the App Store or Google Play and download MyChart or go to MyChart.com to setup an account.
  2. Choose Texas, for where you receive care, or if you are on your phone app, allow current location while using app, then choose CommUnityCare.
  3. Request an Activation Code. Enter your name, address, email address, and medical record number. Click Submit.
  4. Once your request is approved, you will receive an email or a letter with your Activation Code and instructions on how to activate your MyChart account.
  5. If you have any questions, please contact your clinic.

Existing Patients

Make an Appointment

If you are an established patient and you need to make an appointment, we are now using MyChart, which is part of our new Medical Records system. With MyChart, you do not need to call us to make appointments, check your results, or even communicate with your provider. To log into MyChart and create an account, click HERE. If this is your first time creating a MyChart account, click HERE for step-by-step instructions.

After Hours/Emergencies

If you experience a health problem outside of our regular business hours, our Hancock and Southeast Health and Wellness locations have extended hours for walk-in care. These services are NOT meant for someone experiencing a medical emergency. Hancock is open every day from 8 a.m. to 8 p.m. Southeast Health and Wellness is open Monday through Saturday from 7:15 a.m. to 8 p.m. and is closed on all major holidays.

Southeast Health and Wellness is located at 2901 Montopolis Drive, Austin, TX 78741, 512-978-9901

For non-emergency but urgent medical questions when our clinics are closed, you may call our after-hours answering service at 512-978-9000.

Patient Concerns

Patient safety concerns can be reported to The Joint Commission:

  • At www.jointcommission.org using the “Report a Patient Safety Event” link in the “Action Center” on the home page of the website
  • By fax to 630-792-5636

New Patients

Making A First-Time Appointment

If you are interested in becoming a CommUnityCare patient, there are two ways to make your first appointment:

  1. Online – If you have never been a patient at a CommUnityCare clinic or you have never been to one of our vaccination clinics, click HERE to schedule your first appointment with a CommUnityCare provider. This is the fastest way to schedule an appointment.
  2. Via Phone – You can call our Patient Navigation Center at 512-978-9015 to be registered in our system and to select the location that will become your medical home. Prior to becoming an established patient, you should speak with an eligibility specialist to determine which programs you qualify for. You can do this by calling our eligibility team at 512-978-8130. As a reminder, our primary focus is to serve the uninsured, underinsured, homeless and the most vulnerable. Please keep that in mind when trying to establish care with us. If you seek care at another health center or hospital, please provide them with the name of your CommUnityCare provider and phone number. Click HERE for a listing of our clinics.
