Making An Appointment
If you are interested in becoming a CommUnityCare patient, please call our Patient Navigation Center at 512-978-9015 to be registered in our system and to select the location that will become your medical home. Prior to becoming an established patient, you should speak with an eligibility specialist to determine which programs you qualify for, you can do this by calling our eligibility team at 512-978-8130. As a reminder, our primary focus is to serve the uninsured, underinsured, homeless and the most vulnerable, please keep that in mind when trying to establish care with us. At this time we are NOT accepting new patients for the purpose of getting the COVID-19 vaccine. The vaccines are in limited supply and will be offered to established CUC patients that have been a part of CUC throughout this pandemic.
Once you become an established patient at one of our sites, you may be asked to call that site directly for future appointments. Main numbers for our individual clinic sites can be found under "clinic locations".
If you seek care at another health center or hospital, please provide them with the name of your CommUnityCare provider and phone number. Click here for a listing of our clinics.
If you experience a health problem outside of our regular business hours, our Hancock and Southeast Health and Wellness locations have extended hours for walk-in care. These services are NOT meant for someone experiencing a medical emergency. Hancock is open every day from 8 am to 8 pm. Southeast Health and Wellness is open Monday through Saturday from 7:15am to 8pm, and is closed on all major holidays.
