CommUnityCare provides care to anyone regardless of income, health insurance coverage, or ability to pay for services. Our team of Eligibility and Financial Screening Specialists are available to help you qualify for affordable, high-quality health care.

During your eligibility appointment, you are required to bring the following documents for EACH household member. Our team can help you with application assistance, if needed. To view a detailed list of required documents, click here.

  2. Birth Certificate
  3. Social Security Card
  4. Income Verification
  5. Address Verification
  6. Any Active Insurance Coverage
