Clinical Pharmacists

At CommUnityCare Health Centers, our clinical pharmacists are here to support your health. They work with your doctor to make sure your medicines are working for you and help manage long-term health problems.

A clinical pharmacist is a medicine expert who works with your doctor to:

  • Help manage long-term health conditions like diabetes, high blood pressure, and asthma
  • Teach you about your medicines and how to take them
  • Adjust your medications if needed
  • Help prevent side effects
  • Order lab tests and check your progress

You may see a clinical pharmacist in person, by phone, or by video.

Our pharmacists help with many health issues, including:
