Behavioral Health

What is Primary Care Behavioral Health (PCBH)?

The PCBH model connects behavioral health services with the primary care you receive at CommUnityCare. In this model, you briefly meet with a behavioral health consultant (BHC) while you are at the clinic for a primary care appointment.

What is a Behavioral Health Consultant (BHC)? 

The BHC is not a traditional counselor or therapist, but a member of your primary care team who helps you with problems that affect your overall health. The BHC works closely with your primary care provider to improve health concerns including but not limited to management of diabetes, pain, hypertension, sleep, depression, anxiety, stress, etc. The BHC does not prescribe medications.

What can I expect when I meet with a BHC? 

  • During your first meeting, the BHC will get a current snapshot of your life, stressors and coping skills.
    • The initial meeting with a BHC is approximately 30 minutes.
  • Together, you create a plan and identify personal goals to help better manage what is going on in your life.
  • After your first visit, you may decide to follow up with the BHC a few times to briefly check in about questions, concerns, or challenges you might have in reaching your specific health goal.
  • Many patients receive the help they need after meeting with the BHC in just two or three visits.

What if I want more traditional mental health counseling? 

If you decide that you want traditional mental health counseling, your care team can help connect you with longer-term counseling services in the community.

Dental Health

CommUnityCare Health Centers provides dental coverage to people who are MAP (Medical Assistance Program) card holders, Managed Care Patients, Medicaid patients under the age of 18, or a Medicare beneficiary who is a patient at one of our CommUnityCare health centers. Emergency dentistry and some other services are also available for self-pay patients of our CommUnityCare health center system who meet established income guidelines and have no other dental insurance benefits.

Our services include dental exams, teeth cleaning, fillings, extractions, dentures, dental sealants, and emergency treatment. We do not provide orthodontics (braces), implants, crowns or bridges. Dental health exams are also offered for infants as young as 1 year of age as part of the child’s well medical checkup. This noninvasive “peek” at the infant’s oral structure aids in the early detection of possible dental health problems.

CommUnityCare also operates the School Based Sealant Program for Austin Independent School District Title 1 schools. Through this initiative, participating second graders receive a dental exam, followed by the application of a decay preventive sealant to new permanent teeth.

Health Care for the Homeless

The ARCH Clinic, which opened Oct. 4, 2004, is open 40 hours a week and provides primary medical care to patients on a walk-in basis for everything from minor cuts and bruises to chronic disease treatment and management. Also available are tests for hepatitis, HIV and TB.

Additionally, there are two case managers located at the Salvation Army who are on hand to organize patients’ care by connecting them with services that will be most beneficial for meeting their individual needs. The ARCH Clinic also works in conjunction with a mobile Dental Clinic and the Right to Sight Clinic (RTS) in which services are provided on a weekly basis. Health care for the homeless is also provided via our street medicine team, which goes out in the community three times per week and provides primary care to individuals out on the street who are not able to come to a CommUnityCare health center. Finally, our mobile team also provides onsite care at various community locations to anyone seeking health care but cannot access any of our other locations. This is done in partnership with local schools, churches and other community-based organizations by setting up a temporary clinic in their location.

HIV Prevention

CommUnityCare offers a wide range of options to assist individuals in accessing health care. In addition to accepting Medicare, Medicaid and most insurance plans, CommUnityCare has eligibility specialist who are available to review all options and offer application assistance and enrollment services for the following:

Ryan White HIV/AIDS Program

  • The Ryan White HIV/AIDS Program provides HIV-related services for those persons residing in Travis, Bastrop, Blanco, Burnet, Caldwell, Fayette, Hays, Lee, Llano and Williamson County who meet the income guidelines and requirements and do not have sufficient health care coverage or financial resources.

 AIDS Drug Assistance Program (ADAP)

  • Texas HIV Medication Program (THMP), the official ADAP for the state of Texas, provides medications for the treatment of HIV and its related complications to persons who meet the income guidelines and requirements.

CommUnityCare’s Sliding Fee Scale Program (SFS) 

  • A program that provides health care services on a sliding fee scale based on an individual’s income and household size. Services include primary care, dental, behavioral health, prescription program, specialty care, HIV services, women’s services, health care for the homeless, walk-in and after-hours care, and pediatric care.

Medical Access Program (MAP) and MAP BASIC

  • Program provided by Central Health for qualifying individuals residing in Travis County. MAP that covers primary care, prescriptions, dental, specialty and hospital care.
  • Note: This covers other health care services outside DPC.

Titles & Grants / ACA Marketplace / Medicaid / CHIP

  • To determine whther you are eligible for one of the programs listed above, you will need to bring documents that show proof of residency, identity and income.

Your First Appointment – What to Expect

Initial visits typically take two to three hours. You will meet with a social worker to discuss available services and your health care needs. You will meet with an eligibility screener to ensure you will have access to all the care and medications needed to keep you healthy. You will also meet your clinical care team, including your provider, nurse and medical assistant.

At the end of your visit, you will leave with a prescription, the medications needed to keep you living your best life, and the knowledge that our team is here for you.

What to bring to your first appointment:

  • All herbal supplements
  • All vitamins
  • All over-the-counter medications
  • Insurance card
  • Driver’s license or picture ID
  • If you are on Medicare and have elected a Medicare Managed Care or Medicare Replacement Plan, bring that card and your Medicare card.

HIV Testing

CommUnityCare is committed to supporting local, state, and national initiatives working to end the HIV epidemic through providing comprehensive HIV prevention and treatment services.

We know that HIV is still on the rise in the south and that 1 out of every 7 people living with HIV are unaware of their status. Testing is the only way to know whether you have HIV.

CommUnityCare conducts routine HIV screening as a part of all new patient appointments. Additionally, screening is available on request. You can schedule by calling any of our locations.

Walk-in HIV screening is available at the following locations:

  • Hancock
  • Southeast Health and Wellness Walk-in
  • David Powell

Pre-exposure prophylaxis, known as PrEP, is a once-daily pill that significantly reduces your risk of acquiring HIV.

CommUnityCare PrEP services are available at multiple locations, and our teams are ready to assist in enrolling patients in prescription assistance programs, including the federal Ready, Set, PrEP program.

Post-exposure prophylaxis, known as PEP, is a treatment regimen that prevents HIV infection after a known exposure to the virus. Having condom-less sex or sharing a needle with a person known to have HIV, or unknown HIV status. It is important to know that the medications used for PEP must be started within 72 hours after the incident occurred to be effective.

Starter Packs: CommUnityCare offers PrEP and PEP medication starter packs that provide immediate access to medication regardless of whether the person has insurance.

HIV Treatment

For close to 30 years, CommUnityCare has provided HIV care and treatment to Central Texans living with HIV. Currently, HIV treatment is available at our David Powell, South Austin (currently not accepting new patients), Southeast Health and Wellness Center, and North Central locations.

CommUnityCare’s trusted health care professionals are committed to helping patients living with HIV live long and healthy lives. Our team includes physicians, nurse practitioners, pharmacists, social workers, dietitians and behavioral health counselors, all of whom are experts in their specialties.

All health center staff members adhere to the highest standards of confidentiality. Personal health information is disclosed only as necessary for treatment, clinic administration (i.e., insurance billing), and as requested by the patient.

Clinic Facilities and Access

CommUnityCare meets all federal, state and local safety and licensing requirements, as well as all Americans with Disabilities Act (ADA) statutes and is therefore readily accessible to those with visual, hearing and mobility disabilities. Interpreters for those with hearing disabilities are provided as necessary, and Spanish speakers are served by clinic staffers who are fluent in Spanish. There is also a translation line service available for any language not spoken by clinic staff.

Mobile Health Teams

CommUnityCare now offers mobile health care. The focus of the mobile health care clinic is to provide primary care and preventive care at locations outside of our own. Through our mobile health clinics, we partner with CBO’s, churches, schools, etc. to bring care to people who otherwise could not access it. These services include:

  • Immunizations
  • Flu shots
  • Physical exams
  • Family planning services
  • Nutritional counseling
  • The diagnosis and treatment of minor illness and chronic illness
  • Specialty care referrals to our community partners
  • Each team will have a medical provider, nurse, case manager, medical admitting clerk and a clinical pharmacist or dietician.

Pediatric

Hear from Pediatrician, Dr. Tara Greendyk from our East Austin Health Clinic about having your child get screened for hearing and vision as part of their regular wellness check.


We provide newborn, infant, and care for teenagers that include well child checkups, immunizations, and screening for health problems that can affect children. We provide many of the same laboratory and diagnostic tests that we have for our adult patients. We also perform vision and hearing screenings, and school, camp and sports physicals.

Primary Care

Physical Exams

We perform complete physical exams including measuring vital signs, a head-to-toe assessment of your body, and discussing and treating problems that might be found.

Family Planning Services

Services include screening and treatment for sexually transmitted infections, education and prevention for STDs and HIV/AIDS, family planning education and counseling, pelvic exams and Pap tests, breast and cervical cancer screenings, contraceptives/birth control, pregnancy testing, counseling, and referrals. For new patient appointments, please contact the patient access center at 978-9015. Title X is a funding source available for you to receive confidential family planning based on your needs. Parental consent is not needed for family planning services to adolescents under Title X.

Diagnosis & Treatment of Illness/Injury

We diagnosis and treat illnesses and injuries that are quick to begin, brief, and oftentimes severe in nature. These types of problems include sprains, sports injuries, worsening asthma, minor cuts to the skin that need stitches, dehydration, bee stings, poison ivy, animal bites, the common cold, flu symptoms, ear infections, pneumonia, and many more. We can also remove moles and skin tags, ingrown toenails, and other minor skin problems.

Diagnosis & Treatment of Chronic Illnesses

We diagnosis and provide treatment of chronic illnesses that require medical supervision and are often diseases that have formed over a long time. Examples are diabetes; asthma; anemia;, thyroid problems; high blood pressure; high cholesterol; heart, kidney and nervous system problems; arthritis; emphysema and other lung problems. We also provide help with quitting smoking.

Diagnostic Studies

We utilize a variety of diagnostic studies including making referrals for X-rays, ultrasound, CT and MRI scans, treadmill testing, echocardiograms, lung function testing and other studies that will help us diagnose and treat your problems.

Laboratory Services

Our laboratory services include blood studies for cholesterol, PSA, blood sugar, kidney function, hepatitis, HIV and many other conditions that can be discovered through blood work. For patients who have been referred, they can visit CommUnityCare labs or go to Quest Diagnostics locations across Austin. Find locations.

Nutrition Counseling

Nutrition counseling is available to help you formulate a good eating plan to help you lose weight, manage your diabetes, and enjoy a healthy diet that helps you maintain your health.

Immunizations & Flu Shots

Immunizations and flu shots are given regularly to help you avoid diseases that can affect your health and the health of your family.

Specialty Referrals

We also can arrange specialty care referrals to specialists such as heart doctors, lung doctors, arthritis specialists, neurology doctors and other medical professionals who are specially trained and can give you more care for your health problem. Your primary care doctor will still be in charge of your general health but these specialists are available should you need them.

Walk-In

CommUnityCare offers walk-in acute primary care services at two convenient locations. We know that sometimes you need prompt attention that can’t wait for a scheduled appointment. CommUnityCare Hancock and the Walk-In Clinic located inside the Southeast Health and Wellness Center were designed to deliver same-day acute care services. Hancock offers walk-in care seven days a week, including holidays and weekends, from 8 a.m. to 8 p.m. The walk-in clinic at Southeast Health and Wellness Center offers services Monday through Saturday from 7:15 a.m. to 8 p.m. During office hours, please contact your home clinic first to see whether we can get you in before going to a walk-in location.

Our goal is to provide immediate care services in support of your home clinic by accepting overflow patients.

Available services include:

  • Cough, cold or sore throat
  • Fever or flu symptoms, vomiting
  • Ear infection
  • Cuts, bumps, and sprains
  • Twisted or sprained ankle
  • General wound care
  • Minor skin rash
  • Mild asthma
  • Basic laboratory testing
  • Minor procedures to include toenail removal
  • Urinary tract infections

If you have a significant headache, chest pain, severe abdominal pain, severe bleeding, or suspicion of a broken bone, check with your provider during the day, or call 911 and/or go directly to an emergency room for care.

Who Can Use the Walk-In Services?

If you have Medicare or Medicaid, you are eligible to use services at these sites. Patients who are already in our system may also use these services. This includes patients with Medicare or Medicaid, those in the Medical Assistance Program (MAP), CHIP and sliding fee schedule patients. Staff members are also on site to help determine whether someone qualifies to become a patient in our system.

How Do I Use the Walk-In Services?

During the Day – Monday through Friday

If you are ill or have a minor injury during normal office hours, please contact your CommUnityCare primary care provider. If that office cannot get you in that day, they will refer you to one of our two walk-in locations.

Weekends and After-Hours
For after-hours or evening illness or minor injury, CommUnityCare patients may go to either the Hancock or the walk-in clinic at Southeast Health and Wellness Center to receive care for an acute but nonemergency need. Patients are seen on a first-come, first-served basis. Our goal is to help you avoid unnecessary trips to the emergency room.

Women's Health

Gynecology services and women’s health exams are available and include Pap smears, breast exams, mammogram referrals, help with menstrual problems, diagnosis of cervical and uterine cancer, hormone therapy, and treatment for osteoporosis. We also talk to you about prevention and treatment of sexually transmitted diseases (STDs) such as chlamydia, herpes, syphilis, hepatitis, gonorrhea, and many more.

Obstetrics and Family Planning

We also assist our patients with family planning, provide pregnancy testing, and care for women throughout their pregnancies.

Consent for Additional Medical Services
Most minors (under the age of 18) are required to obtain consent from a parent or guardian prior to receiving medical services at some of our health centers. If you have a cough, cold or any other illness not related to family planning, you will be asked to provide parental consent. Eligibility requirements must still be met to obtain medical services. Please contact 211 to schedule an appointment with an eligibility screener.

EpicCare Link is a tool that provides real-time web access to patient information so you can access patients’ clinical data and communicate with CommUnityCare to provide quality patient care. You can also use EpicCare Link to quickly refer patients to our organization.

EpicCare Link is a collection of different web pages, or activities, that correspond to different tasks. The activity that you use depends on what you want to accomplish.
