The CommUnityCare Health Center team believes everyone should have the right to healthcare. That’s why CommUnityCare services and programs are available to anyone who needs them, regardless of your ability to pay. We don’t judge anyone. 

CommUnityCare Health Centers accept most insurance plans, like Medicaid, Medicare, and payors such as MAP. There are also specialists at most locations who can help you reduce the cost of services. 

You may apply for financial assistance online or make an in-person appointment with one of our Eligibility Specialists. See below for more information. 

If you don’t have insurance, CommunityCare™ Health Centers can still help you get health care

We know getting healthcare can be confusing especially if you don’t have insurance. Our Eligibility Specialists can help you find ways to make healthcare more affordable. Your eligibility for programs will depend on your income, family size, address, and other criteria.

Eligibility specialists are available at most clinic locations. You can apply for health coverage online or in person.

  1. Start by filling out our online form here.​
  2. Our eligibility specialists will check your application and reach out if we need more information. ​
  3. If our specialist says we need more details, you can click here to upload.​
  4. Our specialist will contact you to confirm if you qualify and tell you where to send your cards. 

    Remember, you have 30 days to finish the application. If not, you’ll need to apply again. 

If you prefer to apply in person, you can make an appointment with an Eligibility Specialist by calling 512-978-9015 or by emailing onlineeligibility@communitycaretx.org.

During the appointment, make sure to bring important documents for each household member: ​ 

  • Photo ID​
  • Birth Certificate​
  • Social Security Card​
  • Income Verification​
  • Address Verification​
  • Any Active Insurance Coverage​

If you do not qualify for financial assistance, you might still get a discounted payment rate based on your income and family size through the Sliding Fee Scale Program. You can schedule a meeting with an Eligibility Specialist to determine your discount rate.
