Coverage
CommUnityCare Health Centers accepts many forms of health and dental insurance plans, including private insurance, Medicare, Medicaid and payors such as MAP. Click here for a complete listing of current insurance and programs accepted.
Please tell us your insurance coverage at each visit so that we can verify that your insurance will cover your CommUnityCare services.
You will be responsible for deductibles or copayments as required by your insurance plan.
View List of Accepted InsurancesWe know getting healthcare can be confusing, especially if you don’t have insurance. Our Financial Eligibility Specialists can help you find ways to make healthcare more affordable. We can help you apply for programs such as MAP/MAP Basic, Medicaid and CHIP or help find if you qualify for government grants to pay for your care. Your eligibility for programs will depend on your income, family size, address, and other criteria. Patients who do not qualify for assistance programs can receive a medical visit for a lower rate based on income and family size. Sliding Fee Scale is determined through a meeting with a Financial Eligibility Specialist.
Financial Eligibility Specialists are available at most clinic locations. You can apply for health coverage online or in person.

- Birth Certificate
- Social Security Card
- Income Verification
- Address Verification
- Any Active Insurance Coverage

Important Information
Each time you see a provider, you will be assessed a charge. Charges may vary from visit to visit depending on:
- How complicated your health problem is
- If you see a CommUnityCare specialist
- If you receive additional services, such as an office procedure
CommUnityCare Health Centers is not a free clinic. We are required by law to collect any fees for services rendered to you which are not covered by your insurance plan or health coverage.
If you do not qualify for financial assistance, you might still get a discounted payment rate based on your income and family size through the Sliding Fee Scale Program. You can schedule a meeting with an Eligibility Specialist to determine your discount rate.

CommUnityCare Health Centers accept various health insurance plans and payors, including:

- CHIP Texas Families
- Medical Access Program (MAP)
- Most marketplaces/commercial insurances.
